Please arrive at Slocum-Dickson 15-30 minutes prior to your first scheduled appointment. Information will be requested for insurance and financial purposes. Please bring any insurance cards or papers with you on your first visit. |
After this initial visit, you will only need to check-in at the designated reception desks for your appointments; however, should any change in your name, address, phone number, marital status, employment, or insurance carrier occur, please notify us promptly.
There are three entrances, Urgent Care, Rehabilitation and the front Main entrance. Wheelchairs are available for patient use while at Slocum-Dickson at each of these entrances.
*Please bring with you all your current medications at each office visit.
